New Patient Time Saver Option: If you are a new patient or have not been to the clinic within the last three (3) years, you may wish to print and complete the following "Registration Forms" and bring them with you to your first appointment. Otherwise, be sure to arrive at least 15 minutes before the time of your scheduled appointment so that you will have time to fill out the necessary paperwork.
